Diem Pham

Founding Partner

Diem wants to empower every caregiver to care for the person within the patient. With an extensive background in acute care pharmacy settings, she has seen and believes all caregivers have the opportunity to uplift and impact every patient and colleague they encounter as though one of their own. Through thoughtful instructions, education and collaboration, caregivers can make use of their expertise and talents to transform prognoses. Education is a passion for Diem evidenced by her founding of the accredited pharmacy education program at a major hospital system and serving as the continuing pharmacy education administrator as part of her position there.

As a young child, Diem was tasked with the safe keeping and timely administration of her grandfather’s medications. Long before she knew dosettes and pill timers existed, she was the medicine dispenser, a critical part of her grandfather’s care. Sadly she watched the many health issues besiege her beloved grandfather. Her short time with him set her on a quest to make a caring difference in every encounter.

In that quest, Diem has served in many facets of pharmacy practices: ambulatory, acute hospital, state institutional, manufacturing, home infusion, and long term care settings, both in Canada and the United States. She is graduate of the University of British Columbia (BS Pharm) and the University of Washington (PharmD), and has extended herself in capacities as staff, clinician, consultant, manager, and director, as well as a patient and a lifelong learner.​
